BILLING SHEET
All Firm Foundation, Inc foster parents are required to submit a billing sheet to his or her case manager. All billing sheets are due by the 1st of each month.
INCIDENT REPORT
All foster parents are required to report all unusual occurrences in the home to their case manager. These reports are to made within 24 hours of the incident.
RESPITE
Firm Foundation, Inc. understands that respite is important and healthy part of fostering. Please submit forms one month prior to respite date.
All foster parents must keep accurate medical administration records for all the children in their home. Please remember all medical records are due on the 1st of each month.
All foster parents are required to submit a reimbursement correction form if there are concerns regarding reimbursement.
All foster parents are required to provide the foster children in their care a copy of the Consumer Handbook and Clients Rights.
All foster parents are required by the state to receive a copy of the Therapeutic/ Family Foster Parent Handbook. This handbook outlines the client rights, foster parent expectations and agency policy and procedures.
All foster parents are required to complete a monthly Fire Drill Exercise. Foster Parents are required to submit a Fire Drill form during monthly documentation drop-off.
It is best practice to complete a Fire Drill Exercise for each new placement.
It is the agency goal to ensure Foster Parents have the necessary resources for weather emergencies.
All foster parents are required to complete a financial assessment during the initial licensure and update their financial assessment every two years during relicensure.
Each foster parent and household members over the age of 18 are required to provide three (3) references, e.g., family friend, co-worker, manager, pastor or anyone who can speak to your character as a person.
Foster parents are to complete a Placement Removal Form if and when placement does not benefit the well being or needs of the child. The agency needs approximately 30 days to make appropriate arrangements for new placment.
